Empowerment Community Services is an OKDHS Certified ADvantage Case Management Agency. Our program coordinates an in-home community based care system and allows frail, at-risk elderly or physically disabled adults to continue independent living. This program is designed to be a community-based alternative to nursing home care. Members must be deemed Medicaid eligible by the Oklahoma Department of Human Services. 

 

We provide home visits to assess needs of the individual and work to protect and empower the health, independence, and well-being of our members. 

 

Examples of ADvantage services our case managers coordinate are:

• Access Ramp or Environmental Modifications

• Adult Day Health

• Bathroom Safety Equipment (ie grab bars, shower chair, hand held shower)

• Home Delivered Meals

• Home Health Care 

• Occupational / Physical Therapy

• Personal Emergency Response System (body alarm)

• Mental Health Services
